HCBS Rent Subsidy Program

Service Description:FINANCIAL ASSISTANCE: provides monthly rent assistance payments to individuals who receive services under a federal Medicaid waiver program and who are at risk of nursing facility placement until they become eligible for any other local, state, or federal rent assistance.

Eligibility:
MUST:
• be 18 years of age or older
• currently be an adult recipient of one of the home-and community-based service waiver program, including HCBS Habilitation Services Program or Money Follows the Person.
• provide evidence that you are responsible for paying more than 30% of your income for rent, and have been determined ineligible, or have been placed on a waiting list, for U.S. Department of Housing and Urban Development (HUD) Housing Choice Voucher or any other available rental subsidy programs.
• live in a qualified rental unit (an apartment, mobile home, or private room for which a signed written rental agreement exists and which is governed by Iowa Code Chapter 562A Uniform Residential Landlord and Tenant Law).
• live in Iowa.

Cannot serve individuals who:
• are receiving federal rental assistance at the time of, or prior to the time of application to the HCBS Rent Subsidy program.
• decline a HUD Housing Choice Voucher or fail to respond to a notice from the public housing authority when a voucher becomes available.
• live in a home owned by a family member or someone who is currently living in the home.

Fees and Method of Payment:No fees

To apply, complete the HCBS Rent Subsidy Application at the bottom of the webpage link. Completed application may be sent by e-mail to HCBSifa@iowafinance.com or by mail, Attn: HCBS Rent Subsidy.

Note: takes applications on an ongoing basis and has a waiting list.

Include the following with your application:
• current written evidence from income sources of monthly income.
• written evidence that the applicant has been determined ineligible, or has been placed on a waiting list, for U.S. Department of Housing and Urban Development (HUD) rental assistance or any other available rental subsidy programs, or a notice from the public housing authority that the wait list is closed (must be signed and dated by housing authority staff member).
• payment information for electronic funds transfer, including bank routing number and bank account number.
